Essential Functions:

• Provides patient care to an assigned group of patients according to the multidisciplinary plan of care.

• Demonstrates the knowledge and skills necessary to provide care to acutely ill patients.

• Facilitates and maintains a safe environment for patients.

• Utilizes the nursing process to deliver patient care.

• Collaborates with interdisciplinary team members to improve patient care.

• Facilitates continuity of care including medication management and patient education.

• Delegates safely and appropriately to other team members.

Job Qualifications:

Education: Graduate of an accredited LPN program/school.

Experience: Preferred: 1 year in a related clinical area.

Licensure: Required: Basic Life Support/CPR, Licensed Practical Nurse.
